Constitution and identity
- Certificate of incorporation / partnership deed / proprietorship declaration; MoA & AoA
- PAN, GST registration, Udyam registration (MSME cases)
- Board resolution authorising the application and signatory
Land and building
- Sale deed / lease deed / allotment letter; mutation record
- Land use conversion order under Section 80, UP Revenue Code 2006 (where agricultural land)
- Approved building plan and completion certificate
- Stamp duty payment challan (for exemption/refund claims)
Project and finance
- Detailed Project Report with means of finance
- Bank / FI sanction letter and disbursement certificate
- CA-certified statement of fixed capital investment with invoice-wise annexure
- Chartered engineer certificate for plant and machinery (and residual life certificate for imported second-hand machinery)
Statutory clearances
- UPPCB Consent to Establish and Consent to Operate
- Factory licence; fire NOC; electricity connection/load sanction
- Sector-specific: FSSAI, CDSCO/Drug licence, DGCA approval, AERB, Legal Metrology
Production and operation
- First commercial sale invoice; commercial production certificate
- GST returns for the claim period
- EPF and ESI records establishing UP-domiciled employment ratio
- Employment register with domicile evidence
Declarations
- Affidavit of no duplicate claim of the same incentive under any other policy
- Undertaking on lock-in period and claw-back
- Declaration that the activity is not on the negative list
